Applicants for registration as a James Lind Alliance Priority Setting Partnership should provide the following details:

1. Names and contact details of partner organisations applying for membership: Please list the names and full contact details of each of the partner organisations supporting this joint application.
2. Annual reports and accounts:
   For each partner organisation of the proposed JLA Priority Setting Partnership, please attach the most recent annual report and statement of accounts.
3. Support from industry:
   Please detail the sources and amounts of support received from industry, and what they have been provided for.
4. Mandate for advocacy:
   Please specify how each of the partner organisations consults its constituencies - of patients and clinicians, respectively.
5. Health problem(s) and treatments about which there is shared uncertainty:
   Please list the health problem(s) and treatments about which the partner organisations share uncertainty, and which they feel would benefit from discussion at one or more JLA meetings, with a view to developing appropriate action plans.
